REQUIREMENTS FOR NEW APPLICATION
Must be personal appearance
I. One(1) copy of duly-accomplished PID application form (filled out in bold/capital letters using a black or blue ink)
II. A. Proof of Identity (PRIMARY IDs)
Bring the Original copy and (1) Photocopy of the following documents, Submit any one (1) of the following:
*Only if no available Primary IDs
II.B. Proof of Identity (SECONDARY IDs)
Submit any two (2) of the secondary identification documents, one of which should bear the applicant’s photo and signature.
III. PROOF OF ADDRESS (must be under the name of the applicant)
For applicant above 18 years old, submit any one (1) of the following documents listed below showing an address within the Philippines. For minor applicants or below 18 years old, the document may be in the name of either father or mother, or legal guardian.
Bring the original copy and (1) photocopy of the following documents, Submit any one (1) of the following:

5. Utility Bill (Cable, Electric, Internet, Landline, Telephones, Water)

2. Certified True Copy of the Marriage Certificate issued by PSA in SECPA, NCIP, or NCMF

2. Certified True Copy of the Divorce Decree duly authenticated by the issuing country's foreign affairs for applicants with a foreign spouse
2. Certificate of Reacquisition of Philippine Citizenship

APPLICATION REQUIREMENTS OF FOREIGN RESIDENTS
Must be a personal appearance and bring the Original copy and (1) Photocopy of the following documents:
I. One(1) copy of duly-accomplished PID application form (filled out in bold/capital letters using a black or blue ink)
II. PROOF OF IDENTITY - Applicants are required to bring both (a) and (b)
a. Certified True Copy and the Original of the Passport – must be valid for at least six (6) months prior to the application
b. Certified True Copy and the Original of any one (1) of the following documents relative to the applicant's stay in the Philippines valid for at least six (6) months prior to the application:
III. PROOF OF ADDRESS (must be under the name of the applicant)
For applicants above 18 years old, submit any one (1) of the following documents listed below showing an address within the Philippines. Please bring the original document and 1 photocopy:

2. If married to a Filipino spouse and the married is solemnized in the Philippines, marriage certificate is SECPA issued by the PSA or by LCR within six (6) months prior to PID applicantion
2. If Filipino or Foreign spouse died in the Philippines: Certified true copy of Death Certificate of the spouse issued by PSA in SECPA or LCR within six (6) months prior to application
2. Certified True Copy of Court Decree of Annulment/Declaration of Nullity of Marriage or any equivalent document issued by a competent authority at the country of annulment duly authenticated by said country's foreign affairs office (if annulled abroad, whether Filipino or foreign spouse)
REQUIREMENTS FOR MINOR APPLICANTS, WHETHER FILIPINO OR FOREIGN
2. Legal guardian, if both parents are abroad or deceased
3. Either grandfather, grandmother, or relative in legal age if both parents are unavailable
2. Certified true copy of the Court Order awarding guardianship or substitute parental authority over the minor applicant under Article 214 and 216 of the Family Code in the following circumstances:
2.1. Both parents are deceased
2.2. If both parents have relinquished their parental authority, the legal guardian must present valid proof of guardianship
REQUIREMENTS FOR RENEWAL WITHOUT CHANGE IN DEMOGRAPHIC DATA
Must be personal appearance
I. One(1) copy of duly-accomplished PID application form (filled out in bold/capital letters using a black or blue ink)
II. Photocopy of Basic or Premium PID Card perforated, if applicable
III. Submit any one (1) of Primary Proof of Identity and/or any two (2) of secondary proof of identity stated in the requirements for Filipinos and Foreigners
IV. Submit any one (1) of the Proof of Address stated in the requirements for Filipinos and Foreigners
REQUIREMENTS FOR RENEWAL/REPLACEMENT DUE TO CHANGE IN DEMOGRAPHIC DATA
Must be personal appearance
I. One(1) copy of duly-accomplished PID application form (filled out in bold/capital letters using a black or blue ink)
II. Photocopy of Basic or Premium PID Card perforated, if applicable
III. Additional requirements to support the changes/amendments, whichever is applicable
REQUIREMENTS FOR REPLACEMENT DUE TO LOSS AND/OR THEFT OF A VALID POSTAL ID
Must be personal appearance
I. One(1) copy of duly-accomplished PID application form (filled out in bold/capital letters using a black or blue ink)
II. Duly-notarized Affidavit of Loss
III. Submit any one (1) of Primary Proof of Identity and/or any two (2) of secondary proof of identity stated in the requirements for Filipinos and Foreigners
IV. Submit any one (1) of the Proof of Address stated in the requirements for Filipinos and Foreigners
REPLACEMENT DUE TO CHANGE OF DEMOGRAPHIC DATA AND/OR DAMAGED DUE TO PHLPOST ERROR/HANDLING
I. POSTAL ID
II. Request Letter for the replacement of PID indicating the correct demographic data
III. Submit to PID PMO Team, Postal ID Payment Delivery Division (PPDD), Business Lines Department for verification and evaluation. Once approved or disapproved the PID PMO Team will notify the requesting party
REQUIREMENTS FOR EXCEPTION CASES, ADJUDICATION/ON HOLD RECORDS
1. Same person but with different names
UNDER ARTICLE 370 OF THE NEW CIVIL CODE OF THE PHILIPPINES, MARRIED FEMALE APPLICANTS HAVE THE LEGAL OPTION TO USE ANY OF THE FOLLOWING NAME FORMATS
1. Her maiden first, middle and surname with her spouse's surname (e.g. Juana Dela Cruz Magat-Pendimar)
2. Her maiden first name and her spouse's surname (e.g. Juana Magat Pendimar)
3. Her spouse's full name, but prefixing a word indicating the she is his wife (e.g. Mrs. Pedro Darco Pendimar)
4. Her Maiden First Name and Surname (e.g. Juana Dela Cruz Pendimar)
MARRIED FEMALE APPLICANTS MAY CHOOSE THE NAME TO BE REFLECTED ON THEIR POSTAL ID, GIVEN THAT IT IS THE SAME AS THE NAME ON THE IDENTIFICATION DOCUMENTS
MARRIED FEMALE APPLICANTS WHO MARRY A FOREIGN NATIONAL ABROAD MUST FILE A REPORT OF MARRIAGE WITH THE PHILIPPINE EMBASSY OR CONSULATE IN THE COUNTRY WHERE THE MARRIAGE TOOK PLACE. THIS MUST BE DONE WITHIN ONE YEAR TO ENSURE THE MARRIAGE IS OFFICIALLY RECORDED WITH THE PHILIPPINE GOVERNMENT AND REGISTERED WITH THE PSA
POSTAL ID APPLICATION DRESS CODE FOR APPLICANTS
To ensure smooth and proper processing of your Postal ID application, all applicants are requested to observe the following dress code during photo and biometric capturing:
1. Wear decent and presentable clothing. Sleeveless shirts, sando, spaghetti straps, see-through clothing, and excessively revealing attire are not allowed.
2. Avoid wearing hats, caps, headbands, or sunglasses during photo capturing, unless required for religious or medical purposes.
3. Earrings, large accessories, or flashy jewelry that may obstruct facial features should be removed.
4. Hair must be neatly groomed and should not cover the face. Eyes and both ears must be clearly visible.
5. Face masks and other facial coverings must be removed temporarily during photo capturing.
This dress code is required to maintain the quality, uniformity, and integrity of the Postal ID as a secure government-issued identification card.

- Marriage Certificate for Married Women (Only if documents and IDs submitted does not bear married name)

II.B. Proof of Identity (SECONDARY IDs)
Submit any two (2) of the secondary identification documents, one of which should bear the applicant’s photo and signature.
- BIR ID paper or digitized/Tax Identification Number card
- Baptismal Certificate
- Certificate of Birth from the National Commission for Indigenous Peoples (NCIP) for Filipinos belonging to indigenous people
- Certificate of Birth from the National Commission for Muslim Filipinos (NCMF) for Muslim Filipinos
- Certificate of Marriage from the NCMF for Muslim Filipinos
- College or Post-Graduate Transcript of Records with readable seal
- Confirmation Certificate
- Elementary or High School Form 137 with readable dry seal for applicants who are 18 years old and below
- Senior High School Form 137 or any equivalent document with readable dry seal
- Marriage Certificate in SECPA issued by PSA for those male and female married applicants
- Marriage Certificate issued by LCR for those male and female married applicants
- Valid Alumni ID
- Valid College School or University ID
- Valid Company ID
- Valid Integrated Bar of the Philippines
- Valid NBI Clearance bearing digited photo
- Valid OWWA ID
- Valid Pag-ibig ID
- Valid Philhealth ID
- Valid PRC ID
- Valid Police Clearance/ID with or without digitized photo
- Valid Seaman’s Book
- Valid Senior Citizen ID

For applicant above 18 years old, submit any one (1) of the following documents listed below showing an address within the Philippines. For minor applicants or below 18 years old, the document may be in the name of either father or mother, or legal guardian.

- Original Barangay Certificate of Residency issued within three (3 months) prior to PID application
- Certified True Copy and the Original of the notarized Land or House Lease Contract
- Certified True Copy and the Original of the Land Title issued by the Land Registration Authority (LRA)
- Certified True Copy and the original of any one Billing Statement or Statement of Account, including but not limited to:

- For widowed applicants
- If married and annulled in the Philippines

- Student Visa
- PEZA Visa
- Any equivalent document allowing the applicant to stay in the Philippines for three (3) months or more, issued by the Bureau of Immigration and/or Department of Foreign Affairs
III. PROOF OF ADDRESS (must be under the name of the applicant)
For applicants above 18 years old, submit any one (1) of the following documents listed below showing an address within the Philippines. Please bring the original document and 1 photocopy:
- Original Barangay Certificate of Residency – issued within three (3) months prior to PID application
- Original Certification from hotel, transient home or any temporary residence, issued within three (3) months prior to application
- Certified True Copy and the Original of the Notarized Land, House, or Condominium, if applicable, for those renting/leasing the property at the address indicated in the application
- Certified True Copy and the Original of any one Billing Statement or Statement of Account, including but not limited to:

- From Married to Annulled in the Philippines and Foreign applicant submit the documents stated in the requirements
- From Married to Divorced: For Filipinos and Foreign applicants, submit the documents stated in the requirements

- Certification by Barangay Captain
- Valid NBI Clearance
- Surrender the already printed PID card
- Court Declaratory Decree
- Affidavit of two (2) disinterested persons not related to the cardholder
- Valid IDs of the two (2) affiants with specimen signature
- Affidavit of rightful identity
- Valid NBI Clearance
